How to Get ADHD Testing Covered by Insurance in Fort Worth

A formal ADHD evaluation is one of the more expensive services in outpatient mental health — comprehensive adult batteries typically run $1,500 to $3,500 in the Fort Worth market, and children's evaluations can go higher when learning-disability testing is included. The good news is that insurance covers ADHD testing more often than most patients realize. The bad news is that getting it covered requires knowing which CPT codes to ask about and which diagnostic language to use when calling your carrier.

The CPT codes that matter for ADHD testing are 96130, 96131 (psychological testing evaluation services), and 96136, 96137 (test administration and scoring). A comprehensive adult ADHD evaluation typically bills 6 to 10 total hours across those codes. When you call your insurance carrier, don't ask 'do you cover ADHD testing?' — the phone rep will often say no. Ask instead: 'Do you cover CPT codes 96130, 96131, 96136, and 96137 when billed with an ICD-10 diagnosis of F90.x for a psychological evaluation?' That's the language that gets you an accurate answer.

Coverage patterns by carrier in Fort Worth: BCBS of Texas PPO plans typically cover psychological testing with a copay or coinsurance after the deductible, with pre-authorization required in most cases. Aetna commercial plans generally cover testing with pre-authorization and often require the psychologist to submit a rationale letter first. Cigna (Evernorth) covers testing but is stricter about medical necessity — you may need to have tried therapy for six months first, or have documentation from a physician recommending the evaluation. United/Optum covers testing on most commercial plans but frequently requires pre-authorization and denies claims that don't include specific 'medical necessity' language.
